Key Responsibilities
- Provide treatment, rehabilitation, and support services in accordance with the FACT model, which is based on the Program for Assertive Community Treatment (PACT) manual.
- Assure the accuracy, completeness, and confidentiality of clinical records.
- Conduct mental health status assessments.
- Coordinate referrals to community providers and support services.
- Provide assistance with activities of daily living, transportation, communication, social and interpersonal relationship skills.
- Provide education related to symptoms, medications, and mental health issues.

Requirements
- Bachelor's degree in Human Services or a related field.
- Previous experience working with adults with severe and persistent mental illness required.
- Valid Florida Driver's License and automobile insurance coverage equal to or exceeding 50,000/100,000/50,000 split limits.
