RESPONSIBILITIES
- Assist residents in securing and maintaining entitlements (e.g., SSI/SSD, PA Medicaid, SNAP, Section 8) and provide referrals for unmet service needs.
- Support residents in developing and implementing long-range service plans in collaboration with the supervisor and staff.
- Conduct weekly home visits and phone contacts, providing additional crisis support as needed, with appropriate documentation.
- Maintain accurate and current records, including resident files, phone logs, and documentation of resident interactions and collateral service provider contacts.
- Notify the supervisor of crisis situations and document incidents accordingly.
- Conduct bi-monthly fire drills and assist with resident move-ins and move-outs, ensuring living spaces are clean and prepared.
- Promote independent living skills such as budgeting, cooking, personal hygiene, and public transportation use.
- Provide transportation for residents to mental health programs, court, and emergency medical services as needed.
- Facilitate resident support groups and connect residents to external educational and vocational resources.
- Participate in staff meetings, supervision, training, and interagency collaborations.
- Conduct monthly rent reviews with the supervisor and assist in discharge planning.
- Ensure program compliance with record-keeping, site conditions, and funding requirements (OMH, HUD, etc.).
- Maintain positive relationships with neighbors and the community.
- Conduct outreach to State Operated Hospitals and Community Residences.
- Rotate on-call duties and participate in committees and professional groups as related to the position.
- Perform additional related responsibilities as needed.
POSITION RELATIONSHIPS
Reports to: Associate Director of Supportive Housing
Supervises: Housing Specialists, Community Services Specialist, Interns
QUALIFICATIONS
- Master's Degree in Social Work or related Human Services field preferred OR
- Bachelor's Degree in Social Work or related field, AND Human Services experience with people living with mental illness required
- CASAC and SIFI certification preferred
- Supervisory Experience Required
- Experience with people living with mental illness, MICA
- Valid NYS Driver's License with a safe driving record, access to a personal and reliable vehicle
- Physical Requirements: Sits, stands, stoops, squats, walks, and moves intermittently during work hours, may push or pull 10-15 pounds daily, may push or pull up to 50 pounds occasionally, drive agency vehicle
*Training will be completed Monday-Friday, 8:30am - 4:30pm*
SCHEDULE: Tuesday - Saturday, 8:30am - 4:30pm
PAY RATE: Up to $51,500 per year ($26.41 hourly)
