Job Description
SUPERVISOR: Reporting to Medical Director
STATUS: hourly, non-Exempt
EDUCATION: Family Nurse Practitioner
SALARY: $55—60 hourly based on experience
POSITION SUMMARY: The family nurse practitioner is a licensed independent medical provider who is responsible for managing health problems and coordinating the medical care of patients in accordance with state and federal rules, regulations and standards of care (health status assessment, diagnosis, development of a care and treatment plan, implementation of a treatment plan and assessment of the patient's condition). The clinical management of patients is conducted in collaboration with other members of the healthcare team.
DUTIES:
- Functions independently to perform evaluations of medical history and physical examinations for complex acute, and chronically ill patients.
- Orders and interprets diagnostic and therapeutic tests
- Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
- Implements and follows through on changes in patients' plan of care as needed.
- Monitors the effectiveness of interventions and adjusts the plan of care accordingly.
- Collaborates with internal and external multidisciplinary team members by making appropriate referrals.
- Facilitates staff, patient, and family decision-making by providing educational tools.
- May be asked to support other providers in related activities, including lab reviews.
QUALIFICATIONS:
- Graduate of an accredited school of nursing or other relevant training
- Graduate of an accredited Nurse Practitioner program
- Current provider card in Basic Life Support (BLS) PALS provider card as required by department and facility
- DEA/CDS
- EHR/EMR, outlook email
SKILLS: Minimum of 2 years of experience seeing patients in a clinical setting preferred.
