What are the responsibilities and job description for the Wellness Outreach Coordinator position at Meritus Medical Center Inc?
The wellness coordinator will spend the majority of time completing outreach phone calls to promote patient wellness and assist primary care practices in improving quality measure outcomes and preventative care services. Will educate patients regarding eligible preventive care services including Medicare annual wellness visits. Will work collaboratively with care team members to achieve an integrated approach to promote, maintain and improve the health of patients. Will assist in developing improved processes for addressing care gap opportunities using methods including electronic medical record documentation as well as presenting during practice staff meetings when needed.
They function to support the care management roles within practices with secretarial and general office work, promoting patient preventative care services, and other tasks to help meet goals of our population health initiatives.
Possessing a positive attitude and ability to function in a flexible and collaborative team is required.
Education - Graduation from high school or the equivalent required. Associate Degree preferred.
Experience - Previous clerical experience in healthcare environment preferred. Experience with office equipment to include Microsoft Office, facsimile, and electronic medical record experience preferred.
Licensure/Certification - N/A
Knowledge/Skills/Abilities - Effective communication and computer skills required. Phone communication skills required - including text. Basic understanding of medical terminology preferred. Ability to prioritize work and multi-task to achieve desired results required.
