What are the responsibilities and job description for the Community Activities Driver position at Merry Glen Home?

· Check daily schedule for latest staffing information and group assignment(s). Attend report with outgoing shift for latest conditions of the Loved Ones and their activity schedules.
· Review prior logging and announcements in Therap and logbooks to keep well-informed of new developments and changes in each resident’s status and service needs.
· Help prepare Loved Ones for their planned outings. This may include toileting, bathing, dressing, grooming, oral care, transfers and repositioning for wheelchair users, etc.
· Operate Merry Glen vehicles to accompany and assist Loved Ones with community activities such as doctor appointments, shopping, attending classes, eating out, recreational events, visiting friends/family, etc.
· Facilitate opportunities for Loved Ones to have positive interactions and experiences while out in the community. Work with the Client Support Manager to research places and events each person may want to participate in, according to their interests and goals.
· Thoroughly document activities upon return to ensure a complete record is available to reference for goal tracking, medical history, service delivery reporting to the state, etc.
· Develop strong knowledge in the specific care methods for each individual; examples include but are not limited to preferred routine for showering, safe transferring as recommended by the physical therapist, cutting food to proper size for eating, etc.
· Familiarity with each Loved One’s care plan, individual instruction and support plan, and if applicable, behavior support plan. Assist them in achieving their personal goals for increasing independence and behavioral challenges such as problem solving, using communication to express their needs and feelings, anger management, etc.
· Familiarity with and willingness to cooperatively assist the team with Direct Support Professional duties between activities is required.
· Keep vehicles clean, maintained and stocked with emergency supplies as needed. Keep van(s) fueled, fill at the end of your work week and never leave a van at less than half a tank.
· Cooperatively support family/guardian involvement and relationships with their Loved Ones, as well as professional communication and information sharing in compliance with HIPAA.
· Accurately and thoroughly chart Loved Ones’ activities, care, observations and other events as required by their plans and contractual obligations via electronic tracking tool. On rare occasions, paper logging may be necessary.
· Mandatory reporting of incidents may be necessary (e.g., GER, calling in to RCS, etc.). Notify on-call leadership, regulatory agencies and law enforcement, as necessary, of issues/problems as required by contract and law.
· Assist in other duties as needed and/or assigned.
Job Types: Full-time, Part-time
Pay: $24.00 - $26.50 per hour
Expected hours: 16 – 40 per week
