Key Responsibilities
Patient Care:
- Develop and manage personalized care plans for home health patients.
- Perform three Start of Care (SOC) visits daily, ensuring timely and accurate documentation.
Team Collaboration:
- Work closely with a multidisciplinary team to provide seamless, coordinated care.
Documentation and Technology:
- Maintain accurate and up-to-date patient records using EMR systems.
- Utilize tablets, mobile devices, and other technology for efficient documentation and communication.
Time Management:
- Effectively manage your daily schedule to meet patient and organizational goals.
Schedule and Shift Details
- Work Schedule: Flexible 4x10 schedule, allowing you to work either Monday–Thursday or Tuesday–Friday.
Qualifications
Education:
- Associate degree or higher in Nursing.
Experience:
- At least 1 year of experience with EMR systems, including platforms like Practice Partner EMR.
Licenses and Certifications:
- Active RN license in Pennsylvania.
- Basic Life Support (BLS) certification from the American Heart Association (AHA).
- Advanced Cardiac Life Support (ACLS) certification from the AHA.
Transportation:
- Valid driver’s license and reliable transportation for patient visits.
