Key Responsibilities
Patient Care:
- Deliver and oversee individualized care plans for home health patients.
- Perform 3 Start of Care (SOC) visits per day, ensuring timely and accurate documentation.
Team Collaboration:
- Collaborate with a multidisciplinary team to deliver coordinated, effective care.
Documentation and Technology Use:
- Maintain precise and up-to-date patient records using EMR systems.
- Use tablets, mobile devices, and other technology for communication and documentation.
Time Management:
- Manage daily schedules efficiently to meet patient and organizational goals.
Schedule and Shift Details
- Hours: 4x10 schedule, offering flexibility to work Monday–Thursday or Tuesday–Friday.
Qualifications Required
Education:
- Associate degree or higher in Nursing.
Experience:
- At least 1 year of experience working with EMR systems, including platforms like Practice Partner EMR.
Licenses and Certifications:
- Active RN license in Pennsylvania (PA).
- American Heart Association (AHA) Basic Life Support (BLS).
- AHA Advanced Cardiac Life Support (ACLS).
Transportation:
- Valid driver’s license and reliable transportation for patient visits.
