Key Responsibilities:
- Manage and deliver personalized care plans
- Collaborate with a multidisciplinary care team
- Deliver high-quality home health services
- Meet charting deadlines
- Self-manage your time and daily patient schedule
- Utilize tablets, mobile phones, and EMR software
Schedule and Shift Details:
- Monday – Friday, 8-hour shifts
- On-call:
- 1 evening per week (5:00 PM – 10:00 PM)
- 1 weekend per month (Saturday & Sunday)
Qualifications (Required):
- Associate degree or higher
- Valid Registered Nurse (RN) license in Pennsylvania (PA)
- Valid Driver’s License (DL)
ANY of the following valid licenses/certifications:
- AHA Basic Life Support (BLS) or
- AHA Advanced Cardiac Life Support (ACLS)
1 years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- Practice Partner EMR
Preferred:
- RN License Number
Job Type: Full-time
Pay: Up to $80,000.00 per year
