Weekend Start of Care Nurse (RN) – Full-Time
Overview
Our award-winning client is seeking a dedicated Weekend Start of Care Nurse (RN) to help deliver comprehensive, coordinated patient care. In this role, you will be responsible for assessing new admissions, implementing care plans, and collaborating with healthcare professionals to ensure patients receive optimal care. The ideal candidate will have an Associate Degree in Nursing, at least one year of hospital-based RN experience (preferably in med-surg, telemetry, ER, or ICU), and a minimum of two years of home health experience. You must be certified in CPR and hold all required licenses and certifications.
Schedule and Compensation
- Work Hours: Four 10-hour days per week, working every Saturday and Sunday plus two weekdays. Preferred schedules include Friday–Monday or Saturday–Tuesday shifts.
- Travel: Occasional travel within the designated area is required, though efforts will be made to maintain a reasonable territory.
- Compensation: Opportunity for a $10,000 bonus for Home Health RN Start of Care.
Key Responsibilities
- Complete start-of-care documentation accurately for new admissions and post-hospitalization patients.
- Evaluate patients for home telemonitoring, integrate telehealth services into the care plan, and notify the telehealth program within established timeframes.
- Collaborate with agency staff to develop and implement comprehensive care plans.
- Monitor and report patient responses to treatments, including any adverse reactions to medications or changes in condition.
- Educate and counsel patients and their families on nursing care, preventive measures, and rehabilitation techniques.
- Assign and guide Home Health Aides by creating written care plans with specific duties for each patient.
- Implement preventive and rehabilitative nursing measures in accordance with agency policies.
- Communicate effectively with supervisors and team members regarding patient concerns using available technology and processes.
- Participate in meetings, conferences, record audits, utilization reviews, performance improvement initiatives, and in-service training sessions.
- Maintain compliance with CEU requirements set by the State Board of Nursing and stay current on reimbursement criteria and documentation standards.
- Adhere strictly to agency policies, compliance plans, and maintain respectful interactions with patients, colleagues, and visitors.
- Submit daily reports to the supervisor or scheduler and promptly report any schedule changes or issues affecting patient visits.
- Optimize daily scheduling to reduce travel time and maintain productivity, aiming for three start-of-care or re-occurring care points per day (six points per week).
- Work holidays as required and complete all documentation on time; failure to do so may result in disciplinary actions.
- Perform additional duties as assigned by the supervisor.
Qualifications
Required:
- Associate Degree or higher in Nursing.
- Valid Registered Nurse (RN) license in Illinois.
- Valid Certified Cardiac Rehabilitation Professional (CCRP) credential.
- Valid American Heart Association certifications for Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
- Valid Regular Driver’s License.
- Must be comfortable with the travel requirements for this role.
Preferred:
- Provide your RN license number (this is required for name clearance).
Orientation Requirements:
- Availability to complete orientation Monday through Friday for at least two weeks, with 8-hour days.
Coverage Area:
- Chicago, Evanston, Skokie, Glenview, Park Ridge, and surrounding regions, with a larger territory due to the focus on start-of-care assessments.
Must have an IL RN license
- Minimum of 1 year of licensed RN experience in a hospital setting (med-surg, telemetry, ER, or ICU preferred)
- 2 years of home health experience
