Key Responsibilities
- Collaborate with the healthcare team to develop and manage individualized patient care plans.
- Provide hospice care and support to patients in Lords Valley and surrounding areas.
- Accurately document patient information and meet critical deadlines for charting.
- Utilize tablets, mobile phones, and EMR software effectively for patient management.
- Efficiently manage time and scheduling to optimize patient care.
Required:
- Associate degree or higher in Nursing.
- Valid Registered Nurse (RN) license in the state of Pennsylvania (PA).
- Minimum of two years of hospice nursing experience.
- At least one year of experience with Electronic Medical Record (EMR) systems (e.g., TruChart EHR or others).
- Valid Regular Driver’s License (DL).
- American Heart Association Basic Life Support (AHA BLS) certification.
- American Heart Association Advanced Cardiac Life Support (AHA ACLS) certification.
- Submission of RN license number for verification.
- Full-time position with flexible scheduling options.
- Local travel required (approximately 20% of your day driving to/from patient locations).
