Job Summary
The Designated Coordinator (DC) is responsible for overseeing and coordinating the delivery of high-quality care and services for clients in compliance with organizational policies, state regulations, and individual care plans. The DC ensures that services align with client needs, promotes independence, and upholds the dignity of those served.
Key Responsibilities
1. Coordination of Services:
- Develop, implement, and monitor individualized service plans (ISPs) for clients.
- Collaborate with caregivers, case managers, clients, and families to ensure effective service delivery.
- Schedule, coordinate, and oversee the day-to-day services provided by direct care staff.
2. Staff Supervision and Training:
- Recruit, train, and supervise direct care staff and ensure compliance with training requirements.
- Conduct regular performance evaluations and provide feedback and guidance to staff.
- Ensure staff adhere to client service plans and organizational policies.
3. Compliance and Reporting:
- Maintain up-to-date documentation on client services, care plans, and staff activities.
- Ensure compliance with state and federal regulations, including licensing and reporting requirements.
- Report incidents, maltreatment allegations, or service gaps promptly to the appropriate authorities.
4. Quality Assurance:
- Conduct regular audits of service delivery to maintain high standards of care.
- Implement corrective actions to address deficiencies in care or compliance.
- Engage in continuous improvement initiatives to enhance client satisfaction.
5. Advocacy and Support:
- Serve as a liaison between clients, families, and service providers.
- Advocate for client needs and ensure their voices are heard in care planning.
- Qualifications
Education and Experience:
- Bachelor’s degree in Social Work, Human Services, or a related field (preferred).
- Minimum of two years of experience in caregiving, service coordination, or a related role.
- Previous supervisory or team management experience is highly desirable.
Certifications and Licenses:
- Certification as a Qualified Developmental Disabilities Professional (QDDP) or equivalent is a plus.
- Valid driver’s license and reliable transportation.
Skills and Competencies:
- Strong organizational and time-management skills.
- Excellent communication and interpersonal abilities.
- Knowledge of state and federal regulations governing caregiving and human services.
- Proficiency in electronic health records (EHR) and documentation systems.
