We are seeking a compassionate and highly motivated Community Outreach Professional & Case Coordinator to join our team. In this role, you will play a vital part in identifying and engaging potential clients in adult day care centers and senior housing communities, as well as providing comprehensive case management services to ensure their access to and adherence to mental health treatment.
Essential Duties and Responsibilities:
- Community Outreach:
- Develop and implement outreach strategies to identify and engage potential clients in adult day care centers and senior housing communities.
- Conduct presentations and workshops to educate staff and residents about mental health services.
- Build and maintain strong relationships with community partners, including social workers, healthcare providers, and other relevant organizations.
- Represent the organization at community events and conferences.
- Case Management:
- Conduct comprehensive assessments of client needs and develop individualized care plans.
- Assist clients with accessing mental health services, including scheduling appointments, coordinating transportation, and providing financial assistance.
- Monitor client progress and make adjustments to care plans as needed.
- Advocate for clients' needs and ensure their rights are protected.
- Maintain accurate and up-to-date client records.
- Provide crisis intervention and support as needed.
