We are seeking a dedicated and compassionate Case Manager to join our BH Palos Hills and Merrionette Park team. In this role, you will provide comprehensive services tailored to a diverse client population, helping enhance their potential and promote optimal functioning within the community. Working in collaboration with a supervisor, you will address emergency and crisis situations within the target population. This position operates across various settings, including office environments, client homes, and community-based locations, ensuring culturally competent service delivery for individuals from diverse backgrounds.

The Case Manager’s key responsibilities include promoting the importance of care coordination to improve clients’ physical, emotional, and behavioral health outcomes. Under general supervision, the Case Manager will utilize intermediate-level expertise to conduct in-depth client assessments, manage more complex cases, and develop customized service plans. Working closely with clients, you will create actionable plans with clear goals and resources, continuously reviewing and updating these plans to meet evolving needs and deliver impactful support.

SALARY: The average starting salary for this position will fall in the range of $40,000 to $46,000 annually. Where candidates fall in this range will be based on skill and experience level. $2,500 sign on bonus ($500 on the Second Paycheck; $1000 at 6 months; and final payment of $1000 on the 12 month anniversary).

ESSENTIAL JOB FUNCTIONS:  

  • Using intermediate level skills, conducts comprehensive client assessment: works with more complex cases.
  • Develops a written service plan with the client to meet client need, specifying goals to be achieved and services to be pursued.
  • Supplements/service plans to meet client needs.
  • As appropriate, provides advocacy, case management, and supportive counseling to individuals, families, and groups in coping with environment and interpersonal relationship issues.
  • Uses expanded knowledge of resources and helps clients utilize them. Works collaboratively with community resources.
  • Handles emergency and crisis situations within target population.
  • Works within contract, grant, and program requirements including meeting timelines and reporting requirements.
  • Reassess and monitors client situation and adequacy of service plan.
  • Collaborates and participates with other program staff in the planning and implementation of service plans.
  • With supervisory approval, authorizes limited amounts of financial assistance to own clients.
  • Identifies systems issues and reports to Program Director as appropriate. May work with community groups and may carry a special community assignment.
  • Takes increasing responsibility for own professional development, utilizing supervision and other developmental opportunities within and outside the agency to enhance skills and understanding.
  • Maintains competencies in working with diverse populations in order to successfully engage clients in assessments, service/care planning and referral and linkage.
  • Completes documentation/data entry on services rendered, attempted contacts, staffing, supervision, etc. in EHR and health population platforms in a timely manner.
  • Works effectively with diverse staff and service populations.
  • Other duties as assigned.

QUALIFICATIONS:

  • Bachelor's Degree in Social Work or related field from an accredited college or university required. 
  • One year of case management experience in a social service or healthcare setting, required.  
  • Crisis stabilization experience preferred. 
  • Bilingual (English/Spanish), preferred. 
  • Demonstrated ability to render adequate service to clients and to organize and manage job efficiently.
  • Must have access to a vehicle, valid driver's license and proof of insurance to perform essential job functions, required. 
  • Requires mobility to work with community-based caseload. 
  • Ability to maintain assigned community and home-based services. 
  • Ability to work effectively with diverse individuals and groups.
  • Ability to work evening and weekend hours as needed.

    PHYSICAL DEMANDS

    • While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch.
    • The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.
    • The employee may occasionally lift and/or move up to 50 pounds.
