Position Description
The Metro Public Health Department (MPHD) is seeking a Care Manager to support families enrolled in the Nashville Strong Babies program (NSB). This five-year federally funded grant program seeks to achieve health equity by reducing infant death disparities that exist across racial and ethnic groups in our community. This position will be housed in the MPHD Population Health Bureau. The Care Manager provides prenatal and postnatal case management to families in Nashville Davidson County. This position is on the front lines of the NSB program, providing home visiting services to new and existing participants. This position reports to the Nurse Manager.
Typical Duties
- Manages assigned caseloads of participants with a high degree of responsibility and expectations, using specialized and technical skill sets and advanced problem-solving experience to assist clients in navigating the NSB program.
- Maintains direct contact with NSB enrollees and potential enrollees; includes conducting daily visit encounters and documenting encounters in a timely manner.
- Escalates any notable changes) in participant status to the Nurse Manager.
- Uses evidence-based home-visiting curricula to deliver education on a variety of topics from prenatal through parenting.
- Follows up with NSB enrollees to connect them to resources and services as needed, documents referrals made, and services received and the outcome of the referrals and services, reports to the Nurse Manager.
- Performs general administrative duties including maintaining accurate documentation of client records, periodic assessments, statistical reports, data entry, emails, and letters.
- Acts as a member of the NSB care team, attending weekly team meetings and case consults with the Nurse Manager and other NSB staff.
Minimum Qualifications
Bachelor’s Degree
Three (3) years of paid, full-time employment in social work, prenatal and postnatal care, or a directly related field.

Valid driver’s license, use of personal vehicle, and maintenance of valid personal vehicle insurance as required by Tennessee Law.
Preferred Experience, Knowledge, Skills, and Abilities
- 1 year of home-visiting or case management experience with a culturally diverse population.
- Bi-lingual Spanish
