Metropolitan Human Services District (MHSD) is one of ten Local Governing Entities (LGE) serving persons with Mental Health Disorders, Addictive Disorders and Intellectual/Developmental Disabilities in Louisiana. Legislated by the State and through its Board of Directors, MHSD is responsible for the operation and management of community-based programs and services for the parishes of Orleans, St. Bernard, and Plaquemines. In mid-2016, Dr. Rochelle Head-Dunham was appointed the Executive Director of Metropolitan Human Services District. Under the executive leadership of Dr. Dunham, MHSD established a new district mission and philosophy, implemented the FY2017 MHSD strategic plan, and engaged in district restructuring. Must be willing to work in jails.

Hospital Navigator Job Description
The Hospital Navigator interacts and provides follow-up services for patients who are currently in the hospital and who will be referred to Metropolitan Humans Service for continuing services upon discharge. The Hospital Navigator collects and maintains accurate, basic patient information relating to registration, scheduling and insurance in compliance with HIPAA guidelines and answers general inquiries about services provided by Metropolitan Humans Services District.

Job Duties
· Completes all appointment scheduling, cancellation, and confirmation requests within scheduling guidelines to provide the first available appointment in conjunction with patient preferences for time, date, and location and documents interactions in the customer relationship management system.
· Accurately completes basic registration, demographic and insurance information, avoiding duplicate accounts, to ensure timely and accurate payment for services while scheduling appointments.
· Appropriately addresses patients’ emergency/urgent needs utilizing and adhering to the MHSD’s policies.
· Assists with training initiatives for new hires.
· Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
· Performs other related duties as assigned.
Education
Hold a high school diploma or GED and one year of experience in healthcare, customer service, or related field with experience working with EPIC or other electronic medical records.
Associates degree, preferred.
