Responsibilities:
- Conducts screening and assessment of high-risk needs, such as substance use, psychiatric symptoms or hospitalization, harm to self/others, abuse/neglect, or domestic violence.
- Provides case management services, including comprehensive care management, care coordination and healthy promotion, comprehensive transitional care, enrollee and family support, referral to community and social supports, use of health information technology to link services
- Provides case management services in a person/family-centered, strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner
- Plans and coordinates case management services, including referral and escort to medical, psychiatric, substance use, crisis intervention, legal, housing, education/employment, entitlements, social, and peer support services
- Maintains engagement with individuals who are often disengaged from care, have difficulty adhering to treatment recommendations, or have a history of homelessness or involvement in the criminal justice system
- Conducts monitoring and follow-up services to promote continued engagement in referred rehabilitative and treatment services.
- Participates in quality improvement activities.
- Performs administrative support activities such as maintaining files and records, printing and duplicating services, purchasing and/or ordering of supplies and equipment and other administrative functions.
- Reviews correspondence and receive and make telephone calls related to cases assigned to respective area.
- Conducts special projects
- Prepares materials for meetings such as agendas, handouts, binders
- Performs responsible departmental program duties in accordance with rules, regulations and grant requirements.
Qualifications:
- LMSW or PLPC and two years of experience with mental health and substance abuse disorder required.
- Licensure as a Social Worker, preferred
- Experience in workforce development, behavioral health, or related field
- Knowledge of motivational interviewing techniques and crisis management strategies
- Familiarity with addiction counseling and treatment modalities
- Strong communication and interpersonal skills
Job Type: Full-time
Pay: $45,000.00 - $70,000.00 per year
Schedule:
- 8 hour shift
- Monday to Friday
