Job Summary
We are seeking a dedicated and skilled Wound Care Nurse Practitioner or Physician Assistant to join our healthcare team of 20 providers serving the rehab and geriatric patients in our community. In this role, you will be responsible for providing comprehensive wound care management to patients with various types of wounds in a skilled nursing or rehab setting. You will work collaboratively with other healthcare professionals to assess, plan, implement, and evaluate patient care. The ideal candidate will have a strong background in patient assessment and medication administration.
Duties
- Conduct thorough patient assessments to determine the nature and extent of wounds.
- Develop individualized care plans based on assessment findings and best practices.
- Administer medications as prescribed and monitor patients for adverse reactions.
- Perform wound care procedures including dressing changes, suturing, and the application of dermal fillers when necessary.
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
- Participate in utilization review processes to enhance the quality of care provided.
- Educate patients and their families about wound care management and prevention strategies.
- Maintain accurate documentation of patient assessments, treatments, and progress notes.
Qualifications
- Current Nurse Practitioner or Physician Assistant license with specialization in wound care or related field.
- Strong knowledge of wound healing processes
- Proficiency in patient assessment techniques and medication administration.
- Ability to perform basic procedures such as wound debridement.
- Excellent communication skills with a focus on patient education.
- Ability to work collaboratively within a healthcare team to provide high-quality patient care.
