Licensed Independent Clinical Social Worker

MFM Health is seeking a passionate, committed Licensed Independent Clinical Social Worker to join our practice. Position would provide telehealth and face-to-face sessions in our Middleton location.

Essential Duties and Responsibilities:

• Under the direction and supervision of the Director of Behavioral Health the LICSW will conduct mental health evaluations and psychosocial assessments, treatment planning and utilization review
• Provide individual and group evidenced-based psychotherapy to patients within a well-established Primary Care Practice
• Maintain up to date, accurate and thorough documentation in the electronic medical records
• Collaborate with multidisciplinary treatment team including PCPs, FNPs and PAs, Clinicians, Social Workers and Care Coordinators or other professionals, within and outside of the organization, to discuss treatment plans and treatment progress and referrals to other community agencies as needed and as appropriate
• Assist in the Management of Crisis Intervention when needed
• In cases of suspected abuse and neglect regarding children, elders or the disabled, notifying the proper authorities following Massachusetts Law and professional and ethical standards of practice
• Participate in continuing education classes, seminars and trainings per license requirements and in accordance with organizational policies
• Participate in regular department meetings as well as individual and group supervision
• Provide supervision to LCSW as needed
• Any other related duties as assigned
  
  

Preferred Experience: At least 2 years of experience working in the field of Social Work and providing direct clinical care with the Behavioral Health population.

Salary: 85k-95k Annually

Education:

• Master’s Degree in Social Work (MSW)
• Independently Licensed (LICSW)
• Minimum of 2 years experience working in a social work role with individuals with Behavioral Health and Substance Use Disorders
