Summary of position:
The Peer Specialist for Disability Services provides peer-based support to individuals with disabilities, fostering empowerment, independence, and recovery-oriented services. Using their lived experience to help individuals navigate resources, build self-advocacy skills, and engage in their communities.
Goals of the position:
· Provide emotional support and mentorship based on lived experience with disability.
· Shall be knowledgeable in different types of disabilities (such as intellectual, physical, sensory, & mental illness).
· Empower individuals to develop self-advocacy and self-determination skills.
· Assist in identifying strengths and setting personal goals for independent living and wellness.
· Advocate for accessibility, inclusion, and rights in various service settings.
Specific Areas of Responsibilities:
Maintain Active Caseload and Person-Centered Care Management for all social determinants of health.
Assist in developing partnerships to enhance disability-inclusive programming.
Help individuals access disability-related services, including healthcare, housing, employment, and education.
Provide information on benefits and county resources.
Support individuals in navigating the application process for services such as Social Security, Medicaid, and vocational rehabilitation.
Maintain accurate records of peer support interactions and service referrals.
Ensure compliance with organizational policies and confidentiality requirements.
Conduct peer-led workshops on topics such as independent living, employment readiness, and self-advocacy.
Facilitate peer support groups for individuals with disabilities.
Promote awareness and reduce stigma around disability through community outreach and education.
All other assigned duties.
Qualifications:
· Shall be knowledgeable of the physical, mental, and social needs of individuals with a disabling condition(s)
· Lived experience with a disability and willingness to share personal recovery journey as appropriate.
· Ability to work independently and as part of a team.
· Strong communication, listening, and problem-solving skills.
· Proficiency in using technology for communication and documentation.
· Bilingual or multilingual skills are preferred.
Valid NJ Drivers License
Educational Requirements
· High school diploma or equivalent
· Certified Recovery Support Practitioner (CRSP) or willingness to obtain certification
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
Expected hours: 35 per week
Schedule:
- 8 hour shift
