Job Description: Provides crisis prevention and management, monitoring, assessment, and service planning and coordination for up to 200 adults with mental illness in the Brazos Valley. Routine Case Management involves accessing medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual. The Routine Recovery Facilitator will negotiate and facilitate the array of services and supports needed to address the client’s personal goals and desired outcomes as identified through the TRR Assessment and Individual Recovery Plan and provide Rehabilitative skills training as needed. The Routine Recovery Facilitator is responsible for ensuring planned services and support are implemented and appropriately documented as well as advocating and ensuring consistent interactions with other agencies. Crisis On Call work required as agency need dictates.
Education: Requires Bachelor’s Degree in Behavioral, Social Science, or Humanities field, Educational Psychology, or Early Childhood Education or Intervention.
Experience: Requires experience working with individuals with mental illness, must be able to work independently as well as with a team. Good organizational skills are necessary.
Hours of Work: 8:00am—5:00pm Monday—Friday with flexibility as needed to meet the needs of clients and med clinic.
Licensure/Certification: Ability to be credentialed as a Qualified Mental Health Professional. Valid Texas driver’s license and are insurable by agency insurance. Continued employment is contingent upon completion of Adult Needs and Strengths Assessment (ANSA) certification as well as Illness, Management and Recovery (IMR) certification.
Physical Requirements: Ability to drive a vehicle; ability to communicate effectively; ability to pass the Preventative management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
