Job description
We are seeking a highly organized and detail-oriented Hospice Scheduler to join our dynamic team. The ideal candidate will have experience in a fast-paced home health setting, working in a multi-physician office, and proficiency with Kinnser software. This role requires previous experience in or combination of experience in handling authorizations, processing referrals, skilled care knowledge, and high census management, as well as some familiarity in insurance verification, intake and eligibility in home health or medical office settings.

Key Responsibilities:
Scheduling:
- Coordinate and schedule patient visits for home health services, ensuring efficient and timely care delivery.
- Manage high patient census and adapt to fast-paced changes in scheduling requirements.
Referrals:
- Process and manage incoming patient referrals from physicians, hospitals, and other healthcare providers.
- Ensure accurate and timely documentation of referrals in the system.
Coordination:
- Collaborate with the clinical team to ensure smooth and efficient patient care coordination.
- Maintain clear and effective communication with physicians, nurses, and other clinical staff.
Documentation:
- Accurately document all scheduling, authorizations, and patient information in Kinnser software.
- Ensure compliance with Medicare regulations and agency policies.
Qualifications:
- Proven experience in a home health setting, preferably in a multi-physician office.
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills.
- Ability to handle a fast-paced environment and manage high patient census.
- Proficiency with WellSky/Kinnser software.
- Experience with insurance verification and eligibility checks.
- Knowledge of Medicare regulations and skilled care services
