Job Summary

The Health and Social Equity (HSE) Analyst will work with CHRT staff and leadership to support CHRT?s growing work focused on health equity and the integration of health and social care services through initiatives powered by information and technology.

The HSE Analyst will support project leads in the development, implementation, and monitoring of community care initiatives aimed at building capacity for health and social service organizations to ensure seamless delivery of services. The analyst will assist in organizing and supporting regular partner meetings to foster collaboration, gather requirements from multiple stakeholders, and contribute to the assessment of program effectiveness by collecting and analyzing quantitative and qualitative data.

This position requires independent management of moderately complex datasets and ensuring data integrity. The analyst will work with project leads to create and document data management and program protocols to ensure compliance with data governance and regulatory requirements. The HSE Analyst will independently prepare comprehensive status reports, synthesizing information from various sources and ensuring clarity, accuracy, and timely delivery of reports while tailoring the content to meet the needs of diverse stakeholders. Reporting includes documentation of program activities, outcomes, and best practices to support knowledge sharing and quality improvement

The HSE Analyst is expected to independently apply knowledge and skills in the subject area to projects and research, contributing to the development of expertise within the team and acting as a resource by synthesizing information and providing detailed analyses and insights. Proficiency with basic technology skills such as Microsoft Office, Outlook, Zoom, and project management tools is expected.

Responsibilities*

60% Data Management and Policy Analytics

  • Use care management platform to support community care coordination program activities that include managing referrals, coordinating external partners, tracking metrics, creating and disseminating reports, and working with IT vendor to improve usability of the platform
  • Work with evaluation team to conduct database management and analysis of program datasets
  • Manage program partner relationships, including communication, metrics reporting, and follow-ups throughout the duration of projects.
  • Lead staff and stakeholder training on IT platform for care management


35% Project Management

  • Work with program manager to support community collaborative projects by planning and conducting root cause analyses, developing project goals and timelines
  • Support project implementation and ensure milestones are completed within established timelines
  • Serve as liaison for external project partners, including healthcare and community-based organizations


5% Communications

  • Work with program manager, communications staff, and leadership as needed to promote external awareness of services and capabilities


Required Qualifications*

  • Bachelor?s degree in public health, health policy, or a related field (with public health or health policy focus). Master?s preferred.
  • Minimum of 1-3 years? related experience and a working knowledge of key public health or health policy issues.
  • Demonstrated interest in population health, health equity, Medicaid policy, health and social care integration, and public health practice.
  • Strong writing and research skills and ability to synthesize complex information for a wide range of audiences.
  • Ability to work independently within defined guidelines, as well as willingness to work in a highly interactive and collegial team.
  • Ability to work collaboratively with diverse stakeholders.
  • Excellent verbal and written communication skills.
  • Highly motivated with a strong work ethic.
  • Excellent organizational skills and attention to detail, including demonstrated ability to manage and prioritize multiple activities and projects.
  • Analyst must be able to attend in-person meetings in Washtenaw and Livingston counties in Michigan as needed to support collaboration and partnership development.


Desired Qualifications*

  • Experience in community health programs or social services
  • Familiarity with HIPAA and Protected Health Information (PHI) best practices
  • Knowledge of social determinants of health (SDOH) and screening practices
  • Familiarity with the design and maintenance of IT platforms for care management


Modes of Work

Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings. Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
