Job descriptionThe Licensed Practical Nurse in a long-term care setting provides support to healthcare providers and nursing staff, assisting with the delivery of comprehensive care to residents. This role involves a mix of clinical and administrative tasks aimed at ensuring the comfort, safety, and well-being of residents.
Key Responsibilities:
- Clinical Support:
- Develop and implement individualized care plans alongside the Charge LPN for long term residents with chronic conditions, collaborating with healthcare providers and family members.
- Assist in obtaining consent from identified patients for the chronic care management program
- Monitor the effectiveness of care plans and adjust as necessary.
- Educate residents and their families about chronic conditions and self-care techniques.
- Resident Care:
- Provide compassionate support and assistance to residents, ensuring their comfort and dignity.
- Monitor residents for changes in condition or behavior and report observations to nursing staff.
- Administrative Duties:
- Maintain accurate and up-to-date medical records, including data entry into electronic medical records (EMR).
- Communication and Coordination:
- Serve as a liaison between residents, families, healthcare providers, and other staff, facilitating clear and effective communication.
- Participate in team meetings and contribute to discussions on resident care and treatment plans.
- Quality Assurance and Compliance:
- Adhere to facility policies, procedures, and safety protocols, including infection control measures.
- Participate in quality improvement initiatives and contribute to a safe and supportive environment.
- Ensure compliance with all relevant regulations and standards.
Qualifications:
- Education:
- Completion of an accredited Licensed Practical Nurse program.
- Certification:
- Licensed Practical Nurse (LPN)
- Experience:
- 1-2 years Previous experience in a healthcare setting, particularly in long-term care, is preferred.
- Skills and Abilities:
- Strong organizational and multitasking skills.
- Excellent communication and interpersonal skills, with a compassionate and empathetic approach to care.
- Ability to work effectively as part of a healthcare team.
- Proficiency in using electronic medical records (EMR) and basic computer applications.
