About the Position
We are seeking an Enhanced Needs Care Coordinator to deliver exceptional needs care coordination, including assessing individual needs and determining service eligibility. This role will collaborate with other organizations to develop operational procedures for local placements, integrating principles of wraparound and person-centered planning.
Main Responsibilities
- Liaison to Hospitals and Other Organizations
- Act as liaison to hospitals and other organizations, facilitating hospitalizations, conducting person-centered planning, and coordinating discharges for clients returning to community living.
- Coordinate closely with crisis teams and emergency responders to track and plan for clients requiring hospitalization.
- Provide transition planning and management for clients moving between settings.
- Development of Operational Procedures
- Develop and implement operational procedures for local placements, including client screening, service planning, and financial management.
- Individual and Family Support
- Deliver exceptional needs care coordination, assessing individual needs and determining service eligibility, and facilitating their transition into the least restrictive environment.
- Community Engagement and Transportation
- Travel locally, regionally, and statewide to transport clients and provide services in the community.
