Teen Outreach Program (TOP) Facilitator

Reports to:  Maternal, Child and Adolescent Health Coordinator

Job Summary

Contribute to mission and vision of MICA by creating a positive, respectful, and safe environment when working with children, families and agency and community partners.  The TOP facilitator is responsible for implementing weekly TOP sessions, collaborating with school staff to coordinate community service learning opportunities, and providing overall support for teens enrolled in the program. Will facilitate the program according to the evidence-based “Wyman’s Teen Outreach Program” approach and support the evaluation and continual quality improvement of the program.  This position must perform regular duties at the assigned worksite. 

Summary of Essential Job Functions

Build and maintain positive relationships with children, families, program staff, and agency and community partners as needed to provide excellent services. 

Maintain the safety and welfare of caseload participants and staff at all times.

  • Must maintain confidentiality of participants and families at all times.
  • Maintain healthy and appropriate relationships with teens, focusing on safety, support, interaction, and engagement.

Compliance

  • Facilitate the Teen Outreach Program in accordance with the training received.
  • Develop implementation plan and facilitate lessons from the Changing Scenes curriculum at weekly TOP clubs.
  • Collaborate with school staff to facilitate planning and implementation of community service learning with teens, ensuring the opportunity for a minimum of 20 hours of service within the 9 month program.
  • Meet with Grant Coordinator (Iowa Department of Health and Human Services) as required.

Documentation and Reporting

  • Maintain accurate and up-to date enrollment forms, attendance records, and service records for all youth as required by Teen Outreach Program standards.
  • Complete reports in accurate and timely fashion.

Develop skills and knowledge related to the position 

  • Complete necessary training to become a Teen Outreach Program facilitator.
  • Must obtain and maintain First Aid and CPR certifications.
  • Must obtain and maintain Mandatory Child Abuse Reporter Training.

  • The person in this position constantly communicates with teens in one-one one and group settings, frequently communicates with community members and colleagues, both one-on-one and in larger groups, regarding agency programs.  Must be able to exchange accurate information in these situations.
  • This position requires constant mental and emotional work. Strong ability to think and reason independently, maintain objectivity and to exercise sound judgment.
    • This position is required to teach and supervise teens in a group setting.
  • The physical environment requires the employee to work indoors, in an office space that may or may not be shared.
  • Frequently operates a computer or other electronic mobile device for purposes of documentation and communication.
  • The person in this position must be able to travel between MICA central office and assigned implementation site.
  • Must have and maintain reliable transportation, a valid driver’s license and proof of automobile insurance.
  • The person in this position occasionally moves about inside the office to access file cabinets, office machinery, etc.

Education & Experience

Required:  Associate’s Degree with 2 years of experience in the human service field

Preferred:  Bachelor’s Degree in Human Services or closely related field
