What You'll Be Doing
To provide safe, compassionate and high quality person centred assessment and case coordination to clients. Ensuring care is delivered in collaboration with clients, their families/carers and a multidisciplinary team to achieve identified goals and outcomes in line with MNCLHD Aged Care Assessment Service guidelines (ACAS).

About you. do you have?
- Qualifications eligible for membership of the Australian Association of Social Workers; or qualifications recognised for registration with the Occupational Therapy Board of Australia; or qualifications recognised for registration with the Physiotherapy Board of Australia; or qualifications eligible for registration with the Psychology Board of Australia; or qualifications eligible for registration with the Nursing and Midwifery Board of Australia.
- Demonstrated ability to conduct comprehensive clinical aged care assessments and problem solve complex cases with a focus on achieving positive client and carer/family outcomes while ensuring a person centred approach within a multidisciplinary team environment.
- Demonstrated knowledge, skills and ability working within an acute and/or community setting including an understanding of the principles of reablement, rehabilitation, Geriatric conditions and chronic disease.
