Job Overview
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care and ensuring optimal outcomes.
Duties
- Conduct patient assessments and develop individualized care plans
- Collaborate with healthcare professionals to provide comprehensive patient care
- Utilize medical terminology to communicate effectively with interdisciplinary teams
- Monitor and manage patient cases, ensuring appropriate utilization of resources
- Apply expertise in areas such as ER, ICU, PICU, and emergency medicine
- Perform utilization management to optimize patient outcomes
- Monitor vital signs and respond promptly to changes in patient condition
Experience
- Must have minimum 2 years of experience in Acute Care Nursing.
- Must have previous experience in Case Management.
- Travel experience preferred.
- NIH Stroke Cert required before start.
Job Types: Full-time, Contract
Pay: $65.00 - $70.00 per hour
