Overview
The Care Coordinator (Licensed Practical Nurse) is a vital role within our Chronic Care Management (CCM) program. This position requires a Licensed Practical Nurse (LPN) with a strong clinical background and a passion for patient care. The primary responsibility is to coordinate care for complex patients with multiple chronic conditions, ensuring optimal health outcomes through comprehensive care management.
Responsibilities:
Patient Care Coordination:
Actively manage a caseload of CCM patients, coordinating care with healthcare providers and ensuring timely follow-up.
Conduct patient assessments to identify needs and develop individualized care plans.
Educate patients and their caregivers on self-management strategies, medication adherence, and disease prevention.
Facilitate timely referrals to specialists and other healthcare services as needed
Monitor patient progress and adjust care plans as necessary.
Data Management and Reporting:
Document patient interactions, care plans, and clinical interventions in the electronic health record (EHR).
Collect and analyze patient data to identify trends and opportunities for improvement.
Generate reports on key performance indicators (KPIs) and outcomes of the CCM program.
Maintain confidentiality of patient information and adhere to HIPAA regulations.
Quality Improvement:
Participate in quality improvement initiatives to enhance the efficiency and effectiveness of the CCM program.
Identify areas for improvement in patient care and coordination.
Contribute to the development and implementation of evidence-based care protocols.
Qualifications:
Licensed Practical Nurse (LPN) licensure in good standing.
Minimum of two years of clinical experience, preferably in a chronic care setting.
Strong understanding of chronic disease management and patient education principles.
Excellent communication and interpersonal skills, including the ability to build rapport with patients and healthcare providers.
Proficiency in electronic health records (EHR) and other relevant software.
Ability to work independently and as part of a team.
Strong organizational and time management skills.
