Position Description
The Family Support Partner will provide support to families with children aged 21 and under who have serious emotional disturbances. The ideal candidate will be highly motivated, empathetic, and able to work effectively with families and other professionals.
Duties and Responsibilities
- Provide peer support to parents/caregivers, focusing on successful engagement, advocacy, and connection to resources.
- Support the implementation of the 10 principles of High Fidelity Wraparound and the 4 phases, ensuring model fidelity.
- Document all interactions and progress accurately and in a timely manner.
- Perform other duties as assigned.
