Coordinator Population Health Program Support - CCCM
Hours: Full-Time/40 hours
Shift Details: Monday-Friday 8am-4:30pm, no weekends required
Position Summary:
The Population Health Coordinator plays an integral role in performing community outreach for the Center for Chronic Care Management to provide education and support to people with chronic disease.
This position provides oversight of the Population Health programs and supports the Center’s day to day operations for multiple patient care services. The successful candidate will interface with patients, physicians, office staff, billing, finance, purchasing and IT to maintain the flow of patient referrals, scheduling and billing. Additionally, it involves participation in program strategy, development and marketing.
Minimum Qualifications:
High School diploma, or equivalent
Must be able to work in a fast-paced environment
Must be computer proficient (Google, Microsoft Office Suite, etc).
Able to multitask and work independently
Must possess excellent communication skills both written and verbal
Preferred Qualifications:
Associate’s Degree or Bachelor's Degree in a health related field
5 years experience in outpatient program support or direct patient care role in a physician office practice, disease management program, homecare or other healthcare setting
