The Middlesex Sheriff’s Office (“MSO”) is seeking a qualified professional for the position of Re-Integration Coordinator to work in the Human Services Department at the Middlesex Jail and House of Correction (“MJHOC”) in Billerica, MA.  Massachusetts Medicaid (“MassHealth”) pays for health care for certain low and medium-income people living in Massachusetts.  The Re-Integration Coordinator (hereinafter “Coordinator”) will be responsible for coordination with case managers, MassHealth and service providers on comprehensive discharge plans to include areas involving medical, mental health, substance use treatment and housing programs for inmates as it relates to MassHealth at the MJHOC.  The Coordinator will ensure all interactions with inmates/detainees and outside agencies for discharge planning are tracked and reported. The hours of work will be 6:00A.M. to 6:00P.M. flex, Monday through Friday.  The hours and days off are subject to change based on the discretion of management and the needs of the Human Services Department.  The Coordinator reports to the Chief of Treatment and Re-Entry Services or their designee.  This is a non-exempt position that will be covered under a collective bargaining agreement (NEPBA, Local 525).

Additional responsibilities of the Coordinator include, but are not limited to, the following:
  • Assisting inmates with MassHealth applications;
  • Verifying, identifying, and managing health insurance information for inmates and detainees committed to the Jail and House of Corrections;
  • Working with community hospitals and MSO accounting to ensure that inpatient stays are billed to the appropriate payee (private or MassHealth) in accordance with state and federal regulations;
  • Contacting either private insurance carriers or MassHealth to verify an inmate’s/detainee’s insurance status;
  • Tracking the inpatient status of both pre-trial & sentenced individuals;
  • Traveling to hospitals and medical facilities to enroll individuals for inpatient services;
  • Contacting the billing department of the appropriate medical facility regarding an inmate’s/detainee’s inpatient status; 
  • Working with MSO medical staff, case managers, and the Senior Re-Entry Specialist to ensure that all inmates and detainees have access to health insurance benefits and, when eligible, are enrolled in MassHealth prior to discharge;
  • Coordinating with MassHealth to suspend and reactivate an inmate/detainee’s benefits in accordance with state and federal regulations;
  • Assisting in reentry issues as determined by the Senior Case Manager.
  • Maintaining accurate statistics for weekly, monthly, quarterly and annual reports and assist the Assistant Superintendent of Program Services and Health Services Administrator in compiling monthly outcome reports as required;
  • Maintaining a working relationship with outside agencies to coordinate care;
  • Coordinating all paperwork for MassHealth applications both inpatient and at the end of sentence;
  • Working with all of the MSO departments including, but not limited to, medical, mental health, MAT, case management, classification and the Electronic Monitoring Program to coordinate appropriate MassHealth planning for community care;


 
  • Coordinating all MassHealth needs according to both the Reentry Demonstration and Consolidated Appropriations Act;
  • Participating in electronic and in person meetings as needed;
  • Working with MassHealth to establish flagging system to ensure all eligible MassHealth members are enrolled for care under the Reentry Demonstration and Consolidated Appropriations Act;
  • Performing other duties as required by management.

Working Conditions:  This position requires:
  • Direct contact with inmates/detainees in a secure correctional setting;
  • Ability to work varied shifts, weekends, nights, holidays and subject to being forced to work over 8 hours depending on the needs of the Health Services Department;
  • Standing and walking for prolonged periods of time;
  • May be required to work alone in isolated areas;
  • Possible exposure to adverse weather conditions;
  • Possible need to lift and/or carry heavy objects up to 100 pounds.
  • Must have valid Driver’s License.

This Re-Integration Coordinator may encounter verbal and physical abuse from offenders while working in a correctional facility and will be required to interact with people who are under physical and/or emotional stress.  The Re-Integration Coordinator must also be cognizant of the confidential and sensitive nature of working in a law enforcement agency and is required to comply with all institutional rules regarding safety and security.

Qualifications: 
  • Bachelor’s Degree, preferred;
  • Strong written, communication, organizational and time-management skills;
  • Ability to keep accurate records and generate timely reports;
  • Valid state Driver’s License.

Additional Preferred Qualifications:
  • Five (5) years of documented experience working with clientele in a human services field;
  • One (1) year minimum working in a correctional setting;
  • Certified Application Counselor for MassHealth;
  • Knowledge of Microsoft Word, Excel, and Access.

In compliance with federal law, all persons hired will be required to verify identity and eligibility to work in the United States and to complete the required employment eligibility verification form upon hire.  The applicant must have an active valid driver’s license, the right to legally operate a motor vehicle in Massachusetts and pass a criminal background check.
