JOB RESPONSIBILITIES

- Develop and maintain relationships with community organizations and resources to facilitate referrals and support for patients.
- Collect and report data on outreach and patient interactions to support program evaluation and improvement.
- Participate in community events and health fairs to promote health education and services.
- Maintain certification and stay current with best practices and guidelines in community health work.
- Collaborate with healthcare providers and other team members to coordinate patient care and support.
- Advocate for patients and help them overcome barriers to accessing healthcare and other services.
