Urgent Hire: We are urgently looking for a reliable 245D Designated Coordinator who is bilingual in English and Somali to join our team!

Position: Designated Coordinator
Hours: 12-20 hours per week - Part time
Salary: $19.00 - $21.00 hourly

Description:

The Designated Coordinator, working under the supervision of the Designated Manager, oversees client management and team member support to ensure overall functionality. This role includes supervising and supporting direct support staff and evaluating client activities. The Designated Coordinator must adhere to Support Plans and comply with federal and state regulations, as well as organizational policies and procedures to guarantee high-quality service delivery.

Responsibilities:

• Assist in conducting client intake, admission, and assessments
• Attend client Support Team meetings as needed
• Review client documents so that they meet licensing guidelines and keep a working knowledge of regulatory requirements
• 245D compliance and paperwork
• Evaluate of the effectiveness of service delivery, methodologies, and progress on the person's outcomes
• Maintain accurate records and reports.
• Review incident and emergency reports, identify incident patterns, and implement corrective actions.
• Serve as a communication liaison with clients, guardians, and other service providers
• Review incident and emergency reports, identification of incident patterns, and implementation of corrective actions
• Serves as a client advocate
• Train, schedule, and provide ongoing support and evaluation of direct care staff

Qualifications:

In order to qualify as the Designated Coordinator as defined in Minnesota Statute Chapter 245D.081, you must meet one of the below requirements:

• A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.
• An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.
• A diploma in a field related to human services, and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.
• A minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the requirements identified in (1) to (3) above.
• Work independently and have strong organizational skills
• Demonstrate strong written and oral communications skills
• Must be respectful and display a positive attitude when working with the clients.
• Must be flexible and adaptable in different environments
• Ability to travel between office to client's home to conduct meetings, as needed.
• Have experience and understanding of 245D compliance
• Pass a Department of Human Services background check (Required)
• Have a current Driver’s License (Required)
• Must be authorized to work in the United States without a sponsor visa
