The MIKID TAY Program is dedicated to providing early transition planning and support for youth aged 13 to 25, particularly those in foster care, DDD (Developmental Disabilities Division), and Title 19 programs. Our mission is to empower young adults by offering guidance, resources, and support as they navigate the transition to independence. We are currently seeking a passionate and dedicated Transitional Youth Specialist to join our team and assist in this important work.
Responsibilities
• Work under the general direction of the Direct Service Manager to implement and oversee transitional youth programs and services.
• Provide one-on-one support and guidance to young adults, helping them develop essential life skills such as budgeting, job readiness, housing navigation, and self-advocacy.
• Collaborate with community organizations, schools, and other stakeholders to ensure comprehensive support and resources for program participants.
• Conduct assessments and develop individualized transition plans for each young adult, setting achievable goals and milestones.
• Facilitate group workshops and educational sessions on topics relevant to transition planning and independent living.
• Maintain accurate records, progress notes, and documentation of services provided to program participants.
• Monitor and evaluate the effectiveness of programs, making recommendations for improvements as needed.
• Stay updated on best practices, policies, and regulations related to transitional youth services and incorporate them into program delivery.
• Transport children as necessary for community and or Center Based Services.
• Other duties as assigned
Requirements
• Associates degree
• One year experience working with youth and young adults, particularly those from diverse backgrounds and/or with special needs.
• Knowledge of child welfare systems, developmental disabilities services, and/or Medicaid programs is highly desirable.
• Strong communication, interpersonal, and organizational skills.
• Ability to work independently as well as collaboratively within a team.
• Compassionate, empathetic, and patient attitude towards supporting young adults in their journey towards independence.
• Proficiency in Microsoft Office Suite and data management software.
• Current Arizona Fingerprint Clearance or the ability to obtain a clearance card.
• Must have valid Arizona Driver's License, business use vehicle insurance and vehicle registration.
• Must complete all necessary paperwork and mandatory training prior providing services.
