Key Responsibilities:
Patient Treatment & Support
- Provide hands-on therapy under the supervision of a Physical Therapist (PT).
- Implement individualized treatment plans to improve strength, balance, endurance, and mobility.
- Train patients in gait techniques, transfer strategies, and the use of assistive devices.
- Educate patients and caregivers on therapeutic exercises and home exercise programs.
Collaboration & Communication
- Work closely with the PT, nursing staff, and interdisciplinary team.
- Document patient progress and communicate updates to the PT.
- Participate in care planning and discharge planning to ensure patient success.
- Educate caregivers and staff on mobility techniques and fall prevention.
Compliance & Professional Development
- Maintain accurate and timely documentation in accordance with facility and regulatory guidelines.
- Ensure treatments align with Medicare and other reimbursement regulations.
- Participate in in-service training and performance improvement programs.
