Key Responsibilities
- Assess client needs and develop individualized plans to achieve their highest level of wellness
- Link clients and their family members with healthcare, and other home or community-based services
- Ensure timely, coordinated access to medically appropriate levels of health and support services
Requirements
- Registered Nurse (RN) with 3 years of clinical experience
- At least 1 year of case management experience preferred
