About This Role
This full-time position requires a registered nurse (RN) with 3 years of clinical experience, including at least 1 year of case management experience.
You will be responsible for direct provision of services that assist patients with achieving their highest level of wellness. You will link clients and their family members with healthcare, and other home or community-based services to ensure timely, coordinated access to medically appropriate levels of health and support services.
Required Skills and Qualifications
- Active and clear license in the State of Florida; maintained once employed
- Strong computer skills, especially in Excel, and knowledge of EMR systems; Athena knowledge preferred but not required
