Job Title: Clinical Pharmacist
Duration: 8 week initial duration (possible extension or conversion)
Job Location: Vacaville CA 95687
Schedule Notes: 7 day stretches working every other week Night shift 7:45 pm to 6:15 am.
Eligibility:
U.S. Citizens, Green Card holders, or individuals with an Employment Authorization Document (EAD) are eligible to apply for this position
Required :
• California Board of Pharmacy licensure required.
Skills & Experience:
• Minimum 1 year of hospital-based experience in unit-dose distribution and IV additives or hospital residency.
• Strong communication skills and knowledge of drug information and therapeutics. Ability to provide pharmaceutical care for geriatric, adult, pediatric, and neonatal patients.
• Physical ability to stand 80% of the shift and lift up to 25 lbs.
Duties:
• Provide clinical judgment and pharmaceutical care to ensure appropriate drug therapy.
• Review drug orders, patient profiles, and medical records for accuracy and appropriateness.
• Compound, prepare, and dispense medications per regulations and protocols.
• Oversee workflow and supervise technicians and interns.
• Consult with medical and nursing staff on drug therapy and provide drug information.
• Maintain accurate patient profiles and perform MAR comparisons.
• Document clinical interventions, formulary compliance, and medication errors in EHR.
• Identify and report adverse drug reactions (ADRs).
• Adjust drug dosages and initiate lab tests under approved protocols.
• Assist with quality improvement projects and data collection.
Education & Licensure:
• PharmD or Bachelor of Science in Pharmacy with continuing education.
• California Board of Pharmacy licensure required
