Position Overview
The Community Health Manager is responsible for overseeing the Community Health Worker (CHW) program within our organization, ensuring that Medi-Cal enrollees receive high-quality, preventive health services in alignment with the CHW preventive benefit. This role will provide leadership and supervision to CHWs, ensuring that care plans are effectively developed, monitored, and executed. The Community Health Manager will play a key role in implementing and refining the CHW program, fostering strong relationships with healthcare providers, and ensuring compliance with Medi-Cal regulations.
Key ResponsibilitiesProgram Leadership & Development
- Develop and implement the Community Health Worker (CHW) program to effectively deliver preventive health services to Medi-Cal enrollees.
- Establish workflows, policies, and procedures to ensure efficient and high-quality service delivery.
- Ensure compliance with Medi-Cal regulations, policies, and reporting requirements for the CHW benefit.
- Collaborate with internal teams and external partners, including healthcare providers, community organizations, and government agencies, to enhance program reach and effectiveness.
Supervision & Staff Management
- Oversee and provide guidance to CHWs, ensuring they deliver care in accordance with best practices and organizational standards.
- Conduct regular team meetings, performance evaluations, and ongoing training to support CHW professional development.
- Provide clinical oversight and ensure CHWs operate within their scope of practice.
- Serve as the primary point of contact for CHWs regarding client concerns, escalations, and care coordination.
Clinical Oversight & Care Coordination
- Supervise the development and implementation of care plans for Medi-Cal enrollees receiving CHW services.
- Ensure appropriate documentation, assessments, and follow-up care are completed for each individual served.
- Work closely with healthcare providers to facilitate seamless care coordination and transitions of care.
- Monitor health outcomes and track key performance indicators to evaluate the effectiveness of CHW interventions.
Community Engagement & Advocacy
- Advocate for CHWs as integral members of the healthcare team in addressing social determinants of health.
- Engage with community partners to strengthen referral pathways and enhance service delivery.
- Represent the organization in public forums, health coalitions, and policy discussions related to CHW services and Medi-Cal benefits.
Qualifications & Requirements
- Licensure: Active Licensed Vocational Nurse (LVN) or Registered Nurse (RN) certification in the State of California.
- Experience: Minimum of 2 years of experience in community health, case management, or a related healthcare setting.
- Supervisory Experience: Previous experience supervising or mentoring healthcare staff, CHWs, or similar roles preferred.
- Knowledge of Medi-Cal: Familiarity with Medi-Cal benefits, especially the CHW preventive benefit and community-based care services.
- Skills:
- Strong leadership and communication skills.
- Ability to develop and implement healthcare programs.
- Proficiency in electronic health records (EHR) and data reporting systems.
- Culturally competent approach to working with diverse populations.
- Other Requirements:
- Valid driver’s license and reliable transportation for field-based supervision.
- Ability to travel across the organization’s service area as needed.
