•Performs social services work providing a full range of information, counseling, assistance, and advocacy for vulnerable and at-risk residents, including unsheltered homeless, veterans, individuals and families impacted by gun violence, and other affected groups.
•Provides assistance and coordination with agencies to help meet individual needs through prompt and consistent service delivery.
•Secures resources for safety planning, intervention, and outreach with residents living in a variety of environments.
•Builds on the strengths of individuals, families, and the community to achieve their recovery and sustainment goals. Performs related duties and responsibilities as assigned.
