- Job Type : Full-time
- Work Schedule : Typically, Monday – Friday 8 : 30-5, must have flexibility to work in the day, evening and occasionally on the weekend.

As a Mobile Outreach Case Manager, you will be responsible for community and street outreach. This includes identifying and building trusting relationships with downtown Minneapolis businesses and homeless youth in the community while assessing the young adults' immediate needs and connecting them with available resources. The main areas of focus are the downtown Minneapolis area, with an emphasis on East, West, North Loop, Mill District, and Loring Park. The Case Manager assists youth with obtaining self-sufficiency through a holistic approach, including but not limited to appropriate referrals for housing, counseling, financial counseling, job readiness programs, and higher education.
