The Human Services of Faribault & Martin Counties has an opening for a full time (40 hours per week) Care Coordinator. This position is located in Blue Earth, MN (Faribault County)

Care Coordinator

We are seeking compassionate professionals committed to ensuring clients receive the most appropriate services, experience smooth transitions between care settings, and maintain clear communication between health plans, county staff, and providers
Examples of Duties
As a Case Manager you will:
- Conduct and lead MnCHOICES assessments and develop personalized Community Support Plans;
- Provide ongoing Home and Community-Based Services (HCBS) case management;
- Lead Pre-Petition Screenings (PPS) and monitor clients on commitment;
- Collaborate with various stakeholders to promote access to primary and preventive care.

Minnesota Merit System Examples of Duties
- Schedules team meetings to review health screenings, new referrals and care coordination activities.
- Provides appropriate outreach, screening, assessment or advocacy
- Performs personal follow up contact with client
- Provides home visits or ongoing short term care coordination
- Performs outreach contacts to current clients to obtain Health Screenings, provide client education and promotes the use of preventative services to improve client health status related to condition management
- Provides support at the local level through education of county staff and providers regarding programs and services available and basic operational information.
- Assists with Quality Improvement Projects and Performance Improvement Projects including outreach activities, data collection and monitoring
- Provides basic technical support and training to other county staff.
Minimum Qualifications
- A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field.
- A bachelor's degree from an accredited four-year college or university with a major in any field and one (1) year of experience as a social worker in a public or private social services agency.
OR
RN:
Current licensure as a Registered Nurse and/or PHN certification in Minnesota.
Examination Process
The examination will consist of a rating of training and experience based on the information you provide in your application and your responses to the supplemental questions included in the application process.
