Job Summary : MNRAAA is seeking a full-time Person-Centered Counselor (PCC) to help Veterans who need assistance choosing their caregivers, services, and supports so they can safely live in their homes. A PCC assesses the functional status of the Veteran and identifies areas of need and preferences to meet those needs. They develop and help implement a care plan that meets the Veterans' identified needs and does not exceed their budget allocation. They perform ongoing monitoring of services and support to ensure the Veteran's needs are met. Requires a valid driver's license, proof of insurance and a reliable car .
Salary and Benefits :
- Salary range : $22.04-$28.94 per hour, depending on years or relevant experience.
