Position Overview
- The LMSW will serve an active role in providing onsite clinical support and therapeutic services to residents within the MOCJ Hotel.
- Functioning as part of an interdisciplinary team of medical, behavioral health and case management professionals, the LMSW will be in regular communication with team members to ensure that residents behavioral health needs are met.
- The LMSW will be responsible for providing and implementing evidence-based, outcome-based interventions, engage in crisis management and address acute mental health/substance use needs of clients engaged in treatment.
- This position reports to the Clinical Supervisorof the program. The LMSW may also perform other duties in support of the agency's quality assurance process.
- Utilize Psychoeducation and Evidence Based approaches (i.e. Problem Solving Treatment; Behavior Activation Treatment; and Motivational Interviewing) in providing patient education, and/or medication adherence to patients treated and/or identified by Primary Care Provider as being in need of psychotropic and/or other form of mental health treatment
- Manages the communication with and between team members to support treatment decision making and/or facilitate changes in treatment recommendations or to support referral of clients to higher levels of care and/or other mental health services; as appropriate.
- Participates in case conferences to advance the on-going coordination of clients' care.
- Conducts and/or promotes diagnostic consultations between the PCP and the Consulting Psychiatrist on difficult to manage case or case where patients are not improving.
- Provides "Self-Management” support/ option to clients who decline coordinated care services but are willing to engage in supported self-management goals.
- Generates timely, accurate, and content substantive progress notes with treatment goals and interventions that are congruent with the clients' diagnosis and consistent with the scope of practice.
- Partakes of one-hour weekly supervision.
- Participate in on-going training and/or certifications as per requirements.
- Master's degree in Social Work and/or Licensed as LMSW in New York State
- Experience in the provision of psychotherapy utilizing various methods of treatment, including evidence-based practice
Salary : $33 - $47
