Position Overview
The LMSW therapist provides specialized Substance Use and Mental Health Treatment Services for a combined Integrated Outpatient Services (IOS License) and Outpatient Substance Use (A32 OASAS 822) panel of care recipients at the East New York Health Center.
The LMSW therapist establishes therapeutic relationships with IOS and OASAS enrollees and provides clinical care through the delivery of individual therapy, groups and integrated collaborations and advocacy to promote optimum wellness with a focus on trauma-informed care, harm reduction and prevention.
The LMSW therapist will provide care to people with diagnoses of mental health and/or substance use from multiple underserved populations, including persons who have ongoing mood and/or perceptual disorders; LGBTQ individuals; the unhoused or unstably housed; persons with chronic health concerns; and persons involved in the criminal justice system.
The LMSW therapist reports to the Assistant Director and is responsible for providing multiple aspects of clinical services to clients through varied clinical assessments of high-risk behaviors, mental health/substance use treatment needs, crisis intervention, individual counseling and group facilitation. The LMSW is responsible for the overall maintenance of a therapeutic environment in the Clinic.

Essential Job Functions
- Maintain confidentiality of records relating to client's treatment
- Completes initial and episodic psychosocial, mental health and substance use assessment, treatment plans, and timely discharges.
- Assess each client for risk level and complete safety planning/monitoring as needed
- Counsel clients, individually and in group sessions, to assist in addressing presenting goals and needs, adjusting to life and making positive changes.
- Complete comprehensive assessments to support appropriate DSM diagnosis
- Act as client advocates to coordinate required services such as detoxification, rehabilitation, respite programs, etc
- Support with emergency problems in crisis situations using de-escalation techniques and clinical engagement with appropriate follow-up and consultation with supervisor and interdisciplinary team.
- Complete all documentation and billing within 24-48 hours
- Engage in outreach to clients who do not attend scheduled appointments and document appropriately
- Collaborate with other staff members in interdisciplinary team to perform clinical assessments and develop and update treatment plans.
- Evaluate client's physical or mental condition based on review of client information, use of prescribed medications and attendance to provider appointments
- Assess the effectiveness of therapy programs and client's progress in resolving identified problems and moving towards defined objectives
- Modify treatment activities and approaches as needed to comply with changes in client's status
- Remains aware of all program policies that ensure quality comprehensive care and to meet the regulatory agencies' guidelines.
- Provides individual counseling and facilitation of varied clinical and psychoeducational weekly groups.
- Perform toxicology assessments as needed.
- Completes weekly, monthly program reports, including familiarity with STARS, HRA-HASA Harm Reduction Report, parole/probation/ACS and collaboration with the courts around client mandates.
- Performs other duties as assigned by Assistant Director of IOS/OASAS services.
- Attend weekly team meetings
- Partakes in regular clinical supervision
- Participate in monthly continuing education
- The Therapist will have access to PHI during the course of his/her work activities. Incumbent will use this information to provide psychiatric therapy as needed in compliance with all regulations and guidelines relative to established plan of care. ALL HIPAA, corporate compliance, and program guidelines must be maintained and kept at all times. Attendance of in-service training required
- Participates in agency wide continuous quality improvement activities aimed at improving health outcomes for program participants
- Attends agency advocacy efforts to assist the continued efforts to end the dual crises of homelessness and AIDS and other agency focuses.
- Master's Degree in Social Work
- Current NYS licensure (LMSW or LCSW) or possesses a permit from the New York State Education Department.
- Minimum 2 years' experience in providing mental health and substance use counseling/therapy within a clinical setting.
- Well versed in diagnosis, therapy, educational programs and development of treatment plans
- Customer service abilities including effective listening skills.
- Critical thinking skills, decisive judgment and the ability to work with minimal supervision in a fast-paced environment.
- Bi-lingual Spanish preferred Competencies The following additional competencies are viewed as important to success in this position
- Must be able to remain in stationary position 50% of time. Frequently move about inside the office to access file cabinets, paperwork, office machinery, retrieve clients from waiting area etc.

- Frequently communicates (exchanges information, converse with, express oneself to) with subordinates, vendors, supervisors, community at large, clients regarding treatment, management, incidents, reporting, advertise services.
- Must be able to determine accuracy of reports (figures, balances, documentation such as case notes) perceive risk, discern agitation of clients and deescalate any situation, judge, observe and assess situations germane to residence/case management operations.
- Excellent verbal and written communications skills; articulation and instruction of policy procedure and practice.
- Frequently exchange information in communication with individuals who are agitated, confrontational or difficult clients and staff from a variety of socioeconomic, cultural and religious background.
- Occasionally move, position or put files weighing up to 10lbs.
- Occasionally required to traverse public transportation in order to attend meetings, give presentations or meet with clients in community.
- Noise level is moderate
- This is a Hybrid role with one or two telehealth days allotted during the week. The essential functions of this position may not be performed remotely. Must operate cell phone or other technology to determine appropriate response to supervisor, co-workers, clients, community partners and other external care providers.
