Title: Masters Level Case Planner
Location: Northeastern Region
Description
The Masters Level Case Planner will provide intensive casework services for children with special medical needs and their families. This role involves collaborating with a multidisciplinary team, conducting outreach, and ensuring the timely delivery of resources and support to achieve permanency goals.
Responsibilities
Promote the organization’s mission, vision, and core values through alignment with policies and procedures.
Collaborate with a dynamic team of professionals, including nurses and social workers, in a foster care program.
Deliver intensive casework services and fieldwork to children, their birth families, and foster families.
Train foster parents in specialized child care skills in partnership with medical staff.
Conduct outreach to connect children and families with community and agency resources.
Maintain accurate written case records and documentation.
Partner with the assigned RN to conduct joint home visits and collaborate on case plans.
Perform additional duties as assigned to support the program.
Qualifications
Bachelor’s degree in Social Work or related field (Master’s degree preferred).
Experience In Foster Care Preferred.
Bilingual (Spanish/English) preferred.
Key Competencies
Excellent verbal and written communication skills.
Strong organizational abilities and flexibility to meet deadlines under pressure.
Proficiency in computer systems and documentation processes.
