What are the responsibilities and job description for the Designated Manager position at MN Care Services?
Job Summary
The Designated Manager is responsible for overseeing and coordinating services for individuals served throughout MN Care Services. They take on the role and responsibility of program management, service coordination, incident reporting, client programming, scheduling, program evaluation, communication and service satisfaction. This includes residential and community-based services to individuals enrolled in a 245D program according to the federal and state regulations. As well as the policies and procedures of the program. The Designated Manager will perform tasks in a way that demonstrates MN Care Services values of effective communication, teamwork, respect, creativity and inclusivity. This position offices in a specific site throughout the metro area.
Essential Responsibilities but not limited to:
1.) Screen referrals for both residential and community-based supports, coordinate site visits and help develop service plans/audits of files for persons served
2.) Travel to program sites, onsite/oversight that involves being highly involved with persons served
3.) Conduct client intakes, admissions, assessments and assure that individualized service plans are developed, implemented, and reviewed/revised as necessary
4.) Attend client support team meetings as needed
5.) Investigate the internal and external reporting of possible abuse and neglect of vulnerable adults and implement corrective action as needed
6.) Oversee Designated Coordinators and assist with development of daily client programming. Instruction and assistance to direct support staff implementing the support plan and the service outcomes, including direct observation of service delivery sufficient to assess staff competency
7.) Communication liaison between guardians and other service providers
8.) Oversee and participate in on-call rotation and assume the responsibilities of a Designated Coordinator whenever necessary
9.) Ensure all program policies and procedures are always current and compliant per licensing requirements. Evaluating the information to develop, document, and implement ongoing program improvements
10.) Maintaining a current understanding of the licensing requirements sufficient to ensure compliance throughout the program. Ensuring corrective action is taken when ordered by the commissioner and that the terms and conditions of the license and any variances are met
11.) Conduct program evaluations and quality assurance reviews
12.) Ensuring staff competency requirements are met according to the requirements. Ensuring staff orientation and training is provided according to the requirements
13.) Participate in staff recruitment, staff evaluations and professional development. Always promote a positive work environment and conduct performance reviews
14.) Ensuring and protecting each person's rights and coordinating Service delivery and evaluation
15.) Evaluation of the effectiveness of service delivery, methodologies, and progress on the person's outcomes based on the measurable and observable criteria for identifying when the desired outcome has been achieved according to the requirements
16.) Evaluation of satisfaction of persons served by the program, the person's legal representative, if any, and the case manager, with the service delivery and progress toward accomplishing outcomes
17.) Other duties as deemed necessary
Qualifications & Must provide proof:
(1) a baccalaureate degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children;
(2) an associate degree in a field related to human services, education, or health and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children;
(3) a diploma in a field related to human services, education, or health from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older or equivalent work experience providing care or education to vulnerable adults or children; or
(4) a minimum of 50 hours of education and training related to human services and disabilities; and
(5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).

· DHS Background Clearance
· Prior experience working in a residential program/setting
· 5 years of prior supervisory/Designated Coordinator experience
· Proficient in Microsoft office software
· Good decision making, good time management, communication skills, flexible schedule
· Experience writing 245D support plans

Pay: $60,000.00 - $75,000.00 per year
