Mobile Health Team Inc., staffing agency has openings for Home Health RN. We are looking for someone with a passion for people.

Assignment Start Date: 2/10/2025

• 13 Week Duration | Days | 5x8-hour shifts | 40hrs GTD

Travel Offer:

• Estimated Weekly Total: $2,440
• Stipend included: $1,400
• Hourly Pay: $26, O.T: N/A

JOB DESCRIPTION

Provides excellent patient care by assisting in collaboration, development, implementation, revision and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items.

• Primary Location: Castle Home Care Services, Kāne‘ohe, HI
• Shift Requirements: 8 Hour Days
• Float Requirements: N/A
• On Call/Call Back Requirements: No
• Weekend: Rotating Weekends as needed (2-3 per month)
• Holidays: As needed
• Unit Information: Home Health - Hawaii Pacific University Campus Annex Building

SUBMISSION REQUIREMENTS

• 2 year Nursing experience - REQUIRED
• 6 months of Home Health experience - REQUIRED
• If they do not have home health experience then we would require at least 2 years of medical surgical or skilled nursing facility.
• Travel experience - REQUIRED
• Active and clear driver's license and car insurance - REQUIRED
• Homecare Home Base and OASIS - REQUIRED
• COVID and FLU vaccine - REQUIRED. Will not take declinations!
• Bachelor's degree - preferred
• Case Manager experience - preferred
• Mileage reimbursed at .43 cents per mile (lower than IRS established rate)
