Responsibilities
- Actively monitors and amends patient visit schedules to maximize productivity and patient needs. Takes action to account for new patients and changes in existing patient schedule, including but not limited to, calling patients to reschedule visits, reshuffling patient load to meet new demands, communicating recommended changes to supervisor, etc.
- Performs comprehensive (OASIS and PT) assessments of assigned patients including, but not limited to psychosocial, educational, safety, and risk factor parameters, as evidenced by clinical documentation and following federal and state regulations, organizational and departmental policies and standards of care, and APTA guidelines.
- Develops a plan of care to promote optimal functional independence and safety as documented by clinical records following federal and state regulations, organizational and departmental policies standards of care, and APTA guidelines. Creates patient-specific goals and ensures that the plan of care reflects logical interventions and advancement towards these goals.
- Plans for and provides skilled physical therapy treatment including, but not limited to, therapeutic techniques, patient/caregiver education, etc. assessing the patient’s status and progress on each visit and ensuring the quality of care as documented in the clinical record and following federal and state regulations, organizational and departmental policies and standards of care and APTA guidelines.
- As Care Manager and/or team member, coordinates patient care with other clinical team members, including but not limited to, physicians, nurses, caregivers, other rehab services, and students, ensuring optimal patient care and communication as noted in the clinical record and compliance with federal and state regulations, organizational and departmental policies and standards of care and APTA guidelines.
- Supervises and directs clinical activities of PTAs, PT students, and home care aides following federal and state regulations, department and organizational policies and standards of care, and APTA guidelines.
- Completes and coordinates required paperwork following federal and state regulations, organizational and departmental policies, standards of care, and APTA guidelines to ensure that documentation reflects the care provided and the outcomes achieved as evidenced in the clinical record.
- Completes required paperwork to ensure charging and billing processes are accurately executed, including, but not limited to, documents that support charges entered, care within insurance coverage parameters, required insurance forms completed, etc. Takes action to address inconsistencies in insurance coverage and desired plan of care requirements. Notifies supervisor of issues as they arise promptly.
- Participates in in-service training programs for our customers including, but not limited to Rehabilitation, Medical, Nursing staff, volunteers, community groups, etc.
- Maintains and enhances personal knowledge as well as promotes standards in physical therapy.
- Proactively engages in the marketing and promotion of Rehab services including, but not limited to, developing specialized areas of service, participating in public health fairs, the medical community, etc.
- Caries out a variety of patient-related health assessments including, but not limited to, PT/INR, blood pressure, etc.
- Carries out a variety of patient-related procedures including, but not limited to, staple/suture removal, wound care, etc.
- Communicate changes in patient care schedules to the scheduler and supervisor promptly.
- Completes OASIS time points following agency, State, and Federal guidelines, regulations, and policies.
- Works collaboratively with QI staff to maximize the accuracy of clinical notes and OASIS documents.
- Follows agency protocol with McKesson processes, including but not limited to, locking-down visits, follow-up notes, visit completion, etc.
- Accurately and fills out a variety of Agency, State, and Federal forms, including but not limited to, HHABN, Medicare Notice of Non-Coverage, etc. promptly. Takes action to ensure that forms are readily available during visits to facilitate timely completion.
- Carries out visits in a time-effective manner. Takes action to ensure that supervisory visits are not combined with routine visits whenever necessary. Takes action to communicate information to the Scheduler promptly.
- As necessary, assesses patients’ needs for HCA services, develops and coordinates POC with patient/caregiver, and provides ongoing supervision by State and Federal regulations.
- Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information is processed according to department policies and procedures, and State and Federal regulations.
- As necessary, transmit patient incident reports (infections, injuries, etc) to QI specialists promptly.
- Fulfills department responsibility to participate in regular peer review audits. Provides constructive feedback and openly accepts feedback professionally.
EXPERIENCE/EDUCATION/QUALIFICATION
- A minimum of 2 years of experience is required
- Vermont PT licensure is required
