Support Coordinator
Job Summary:
Modern Support Services is dedicated to empowering individuals with developmental disabilities to lead fulfilling lives. As a Support Coordinator, you will play a crucial role in providing personalized support and assistance to individuals, focusing on enhancing their independent living skills and fostering community inclusion. Responsibilities:
- Coordinate and provide support services to individuals with developmental disabilities based on individualized plans.
- Coordinate all medical appointments.
- Assist with supported employment tasks
- Assist in developing skills related to self-regulation, decision-making, hygiene, household care, social interactions, and work skills.
- Facilitate community participation and connections for individuals, ensuring their health, safety, and well-being.
- Maintain documentation of progress towards goals and effectiveness of support programs.
- Collaborate with a multidisciplinary team to ensure comprehensive care and support for each individual.
- Demonstrate excellent communication skills and problem-solving abilities in addressing individual needs.
- Uphold the values and mission of Modern Support Services in all interactions and activities.
- Organize and schedule Training of new staff. Organize training by the consultants of new staff. (BCBA, Speech, Music, Communication)
- Organize and get bids for Making Videos of tasks/task analysis and info re Jeremy
- Upload photos of paintings; Upload BIO; Descriptions of paintings; size, price; etc.
- Handle regular scheduling of staff
- Identify and hire New staff as needed.
- Conduct bi-weekly payroll.
- Review monthly spending report for accuracy.
- Research & identify needed resources.
- Meet in person with the clients on a regular basis.
-Coverage as needed.
-On Call Rotation.
Communication and coordinating:
- Coordinate team meetings and follow up with paperwork or tasks identified during the team meeting; send out the team notes and video recording.
- Attend various meetings in collaboration with other agencies.
- Coordinate /communicate with consultants
Experience:
- Previous experience in a human services field is highly desirable.
- Strong organizational skills with attention to detail.
- Proficiency in administrative tasks and office management.
- Bilingual proficiency is a plus.
- Knowledge of customer support principles and practices.
- Familiarity with front desk operations, phone systems, and phone etiquette.
- Computer literacy with the ability to use office software effectively.
- Demonstrated ability in maintaining an organized work environment.

This is an EXEMPT/Salaried POSITION $68k/annual
